This search is for the 'General Register' entry only. To search for both the 'General Register' and more detailed asmission forms, see above. The research fee of £4.00 will include the provision of a digital image of the entry in the 'General Register' (NRS reference MC7) relating to the patient. Although the original record is brief it will contain the date and reason for why the patient left the institution if they left there before death.
